The Intake Care Manager, is the bridge between the older adults in the community and the in-home services that they need to live independently. This is a Local Union 509 full-time, non-exempt (35 hour/week) position. We offer a hybrid remote schedule (1 days/week in the office) and the option of a flexible 4-day work week. Our office is located in Malden, MA. Compensation: Starting annual salary is $48,382.52. Enjoy benefits that start as of day 1 of employment and our generous paid time off package. In this role you will not be providing ongoing case management, but you will be the initial point of contact for applicants. The Intake Care Manager will be responsible for conducting initial assessments and creating the initial service plan, before the consumer is transferred to a Health Services Care Manager for ongoing monitoring. You will also provide information and referral to applicants who do not enroll in MVES programs. Additional duties may be assigned as required. Screen referrals for all programs - determine eligibility according to ASAP regulations and provide information and referral to ineligible applicants Utilizing a person-centered approach to conduct assessments of consumers by telephone, video conferencing, and in-home visits to identify unmet needs. Develop, authorize, and implement an initial care plan by coordinating community-based services with elders, families, caregivers, and service providers to meet identified need(s). Maintain accurate consumer information in case records and client services database in accordance with Executive Office of Elder Affairs (EOEA) and Mystic Valley Elder Services standards. Assist hospitals and nursing facilities with discharge planning. Participate in the interdisciplinary triage of referrals. Ensure that elders have access to public benefits and other community services. Provide crisis case management prevention and intervention and initiate services when immediate intervention is required. Compile and submit required reports. Qualifications: A Bachelor's degree in Social Work or a related field. Social Work license preferred. Knowledge of and experience working with older adults in a community-based setting preferred. Strong interviewing and assessment skills. Strong interpersonal and organizational skills. Excellent written and verbal communication skills. Computer skills: Proficient with MS Office Suite and entering narrative and other data into a database. Ability to use multiple databases. Ability to use the Internet to conduct information searches Private transportation.
