Nurse Supervisor - Personal Care Management Care Program

The Personal Care Management Nurse Supervisor is responsible for the oversight of the administrative, technical and clinical casework supervision of the Personal Care Management (PCM) Registered Nurses (RNs), Occupational Therapist (OT) and Licensed Practical Nurses (LPNs), if applicable. Responsible for initial and ongoing training regarding the PCA Program requirements, specifically how to conduct evaluations and re-evaluations. Ensures that the evaluations and re-evaluations accurately reflect the consumer's functional abilities and need for physical assistance in performing Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL). Monitors and reviews performance improvement activities in medical and nursing functions to ensure that the quality standards of our payer sources are met. Assist in the development and implementation of nursing policy and practices within the PCM department. Acts as the Executive Office of Health and Human Services (EOHHS) contact person for clinical issues and issues related to evaluations. Acts as lead PCM RN for all grievance and appeal issues related to clinical assessments and evaluations. Maintains good working relationship and open communication with doctors and other professionals in the community in order to monitor the health status of each participant. Ensures that all case files are maintained by timely completing all mandated paperwork in accordance with the required documentation standards and relevant policies and procedures. Maintains strong working knowledge of the PCM Program's funding sources, including MassHealth, Senior Care Options (SCO), Accountable Care Organizations (ACO), and OneCare, and understands how each funding source directs the scope and manner in which the PCM Program services are delivered. Implement quality assurance measures to evaluate the integrity of the department's practices including a system for ongoing record and performance reviews. Conduct monthly team meetings. Provide coverage for team/department, including conducting home visits/assessments as needed to ensure regulatory/contractual compliance in the absence of team members. Represent the agency within the community and the service system. Education and Experience Valid MA License to practice as a Registered Nurse Requires a minimum of 3 years nursing experience in a community setting Bachelor's Degree in Nursing Supervisory experience required Public Service Loan Forgiveness (PSLF) If you are employed by a not-for-profit organization such as GSSSI, you may be eligible for the PSLF Program for student loans.
