You'd be joining a team focused on healing our communitiesAcenda is an integrated health care agency dedicated to industry-leading prevention, treatment and wellness services compassionately delivered to ensure that every individual, family and community achieves their greatest potential.What you'll bring to Acenda- Be a leader of a key component of the NJ Acute Care System, overseeing PACT, IFSS, IOC and SEP Teams in Gloucester County.- Increase relationships with community providers in Gloucester County to ensure individuals are being appropriately referred into PACT,IFSS,IOC and SEP Programming.
