POSITION TITLE: LPN Care Manager COMPANY: Suite Living Senior Care LOCATION: West St. Paul, MN SHIFT: Flexible, Days (Standard 8-hour shift, M-F) SALARY RANGE: $70-75k/yr (DOE, Negotiable) REPORTS TO: RN/Clinical Supervisor ***Suite Living Senior Care has 13 Senior Living Communities located across the Twin Cities Metro Area. All of our communities are 32-bed Memory Care + Assisted Living facilities.*** SUMMARY: Suite Living Senior Care is seeking a compassionate LPN Care Manager to join our senior living community located in West St. Paul, MN! Suite Living Senior Care has 13 communities throughout the Twin Cities Metro area- including Anoka/Ramsey, Brooklyn Park, Crystal, Little Canada, Vadnais Heights, Roseville, North St. Paul, West St. Paul, Inver Grove Heights, Spring Lake Park, Shakopee, Burnsville and Prior Lake, MN. This particular role will serve as the Lead Nurse overseeing a 32-bed Memory Care + Assisted Living facility in West St. Paul, MN. JOB DESCRIPTION: The Licensed Practical Nurse (LPN) Care Manager reports to the RN/Clinical Supervisor and is responsible for the provision and monitoring of high-quality clinical care and services to residents consistent with regulations and established clinical best practices. Duties include but not limited to; assisting with admission procedures, assessments, training unlicensed staff, providing medication, treatment administration, and personal care. As the Lead Nurse, you will provide a multitude of cares and responsibilities that include supervision, training & monitoring of unlicensed staff, conducting focused assessments, med passes, auditing, charting, prescreens, admissions and intakes, working with Doctors and Insurance companies in transcribing and ordering medications under the direction of the RN, communicating with clients and families, assist with wound care, skin checks, and other cares as needed. This position is independent in nature, which requires utmost accuracy and discernment in the ability to follow direction and take initiative. Must be comfortable in the supervision and training of all unlicensed staff as they will be passing Meds under your direction. Experience in Memory Care and familiarity with Point Click Care is highly desired. RESPONSIBILITIES: Works under the direction of the RN/Clinical Supervisor to monitor clients' needs and changes in condition, conducting focused assessments when appropriate and reporting the results to the RN/Clinical Supervisor Complete resident Assessments as delegated & reviewed by the RN/Clinical Supervisor Ensures that unlicensed staff members perform their duties and responsibilities in a timely, quality manner, and reports problems to RN Reports changes in client condition or other concerns about clients to the RN in a timely manner Initiates emergency procedures for clients and reports to RN Reviews client care plans developed by the RN and consults with RN about client conditions and their treatments Assists with implementation of physician orders Carries out proper charting practices and documentation procedures Assists with initiation of services for new clients, transfers to settings such as hospitals, nursing homes or assisted living and discharges as requested by RN Supervisor Assembles and operates equipment such as oxygen suppliers and other equipment used for client care Orders, sets up and administers medications Assists RN/Clinical Supervisor in orienting, training and mentoring new staff Attends required meetings, in-service training and conferences Performs delegated tasks and other duties as assigned by the RN Assists the RN/Clinical Supervisor in the monitoring and evaluation of the job performance of unlicensed staff Schedules and assists work of unlicensed staff within established guidelines QUALIFICATIONS: Licensed Practical Nurse with current licensure with the State of Minnesota. Current CPR certification. 1 or more years experience in Long-Term Care highly preferred Interest in gerontological nursing, rehabilitative and restorative clinical practices. Trained Medication Aide (TMA) a plus. PointClickCare experience preferred. Must have excellent judgment, decision-making abilities, problem solving skills, and the ability to work independently Must communicate tactfully and professionally with clients, their families, and all internal staff Must demonstrate leadership and dependability Must be knowledgeable and accurate in the administration of medication and treatments Must be flexible in dealing with changing situations and prioritizing accordingly Must have excellent discernment to comprehend and follow instructions with utmost accuracy Must be knowledgeable about validation techniques, reality orientation and/or appropriate diversional activities for clients with Dementia Must receive a "Not Disqualified" criminal background result from the Department of Human Services and may not be on the OIG Exclusions List Must be screened for TB Job Posted by ApplicantPro
