POSITION SUMMARY: Acts as a charge nurse in the Alzheimer's Care and Research Center. Supervised by the Facility Manager or another nurse designated with supervisory responsibility authority. As directed by supervisor, assists, with planning, organizing, developing, and directing day-to-day facility operations. Participates in resident needs assessments and in the development and coordination of resident care plans. Performs required nursing procedures associated with designation as a Licensed Vocational Nurse. Effectively interacts with resident family members, physicians and other professionals, internally and externally. GENERAL REQUIREMENTS: Comprehensive knowledge of current nursing practices. Must be able to speak, read, write, and follow verbal and written instructions in English. Possess the ability to be tactful and display diplomatic communication techniques in potentially sensitive or emotionally charged situations. EDUCATION AND EXPERIENCE: Required: High school diploma or equivalent. Preferred: Knowledge of current nursing practices related to geriatric and dementia care, as well as hands-on experience with same. EQUIPMENT AND COMPUTER OPERATIONS: Working knowledge of various medical and business equipment, including but not limited to, computer, telephone equipment, facsimile machine, and photocopier. This includes both simple and complex medical equipment as well as patient lifts, beds, call systems, and whirlpool baths. Training will be provided as necessary to familiarize the incumbent with equipment used and to ensure safety during use. Experience with electronic medical records preferred. CERTIFICATES, LICENSES AND REGISTRATIONS: Required: Must be a licensed nurse in the State of Texas and in good standing with the Board of Nurse Examiners. Possess current Cardiopulmonary Resuscitation and AED certification. Preferred: Certification in Long Term Care is desirable. ESSENTIAL DUTIES & RESPONSIBILITIES: 1. Establishes and maintains an environment that recognizes each resident's individuality and rights. 2. Performs duties related to pre-admission, admission, and discharge of residents. 3. Ensures accurate admission information is received and translated to care staff. 4. Reviews all pre-admission materials including physical assessments and cognitive skills assessments. 5. Supports all admissions and discharges and ensures completeness and accuracy of admission/discharge materials. 6. Responsible for medication regime including acceptance of physician's orders, transcribing orders, ordering medications, and ensuring medications are on hand. 7. Administers medications as well as supervises medication aides to ensure accuracy in their duties. 8. Acts as liaison between Freedom House and ancillary support services. 9. Ensures medical supplies are accessible; relates need for additional supplies in a timely manner. 10. Maintains medical records, ensuring necessary records are available and accurate. 11. Practice nursing skills within ones scope of practice, adhering to all state and federal regulations. 12. Maintains a safe environment for residents. 13. Assists in daily living activities as well as in programming activities for residents. 14. Monitors and supervises Personal Care Givers (PCGs), ensuring they are providing appropriate care for residents and meeting their needs. 15. Provides PCGs with guidance and education as needed to ensure residents' needs are met and cognitive capabilities and physical functioning is understood by the care givers present. 16. Responds promptly and properly to all emergencies and disasters, per established Blue Skies policies and procedures. 17. Ensures all caregivers are trained on and aware of their responsibilities in the event of an emergency or disaster. 18. Monitors overall health and welfare of residents. 19. Monitors and ensures all documentation materials are completed and accurate; including materials completed by the Lead Care Givers, PCGs, and Medication Aides. 20. Participates in various Quality Assurance Performance Improvement Committees and their assigned tasks. 21. Participates as a member of the Freedom House Team. 22. Provides pet care as needed. 23. Performs light housekeeping duties as needed, ensuring the safety and wellbeing of residents and their homes. 24. Provides oncoming Lead Care Giver with accurate shift report. 25. Communicates with family members, providing them with information relating to the residents' care and condition. 26. Communicates in a professional manner with physicians, other staff, support services, and outside agencies. 27. Acts as residents' advocate. 28. Responsible for preparing and implementing individualized Service Plans for all residents within 21 days of admission and every six months following. 29. Must recognize changes in condition and reflect such on Service Plans as needed. 30. Must relay Service Plan of care to PCGs and ensure care is provided as planned. 31. Responsible for completing all other assessment tools used at Freedom House as assigned. 32. Responsible for conducting Service Plan Meetings with family members. 33. Attends in-services and training sessions as required. 34. Other duties as assigned by the Facility Manager. SUBORDINATES: Personal Care Givers and Medication Aides INTERNAL CONTROL: Within the scope of position duties, responsible for seeing that operations are effective and efficient, assets are safeguarded, and applicable laws, regulations, policies and procedures are complied with. This position may handle confidential information and must treat such with appropriate sensitivity. PHYSICAL/ENVIRONMENTAL DEMANDS: The physical demands of the job are generally light to medium and involve lifting normally not more than 50 pounds without assistance of another employee and a mechanical lift. Must be able to push a wheelchair a few hundred yards unassisted and be able to bend, squat, reach, climb, and make walking rounds of Freedom House. Incumbent's eyesight must be correctable to 20/20 in order to quickly and accurately assess condition of residents. Incumbent must be able to respond to medical emergencies in work areas. WORK SCHEDULE: Work normally coincides with one of three standard shifts: 6:45am to 3:15pm, 2:45pm to 11:15pm, or 10:45pm to 7:15am. However, exact duty hours are established by the Nursing Manager and the Executive Director. The daily working schedule is developed by the care team members themselves. WORK LOCATION: Blue Skies West, Freedom House. ATTIRE/DRESS CODE: Dark blue or khaki "Dockers" or similar work pants. Polo knit tops with a collar in any color or design. Employees are not to wear white or black tops. Close-toed, slip-resistant shoes required. See employee handbook for specific guidelines. RANGE OF PAY: Dependent upon qualifications and experience, with the intent to be competitive within the market.
