Bilingual Patient Navigator This position provides patient centered services that link refugees with health care, psychosocial and other services to insure timely coordinated access to medically appropriate levels of healthcare and support. This care is to be delivered with compassion and concern. This position will interact with all departments, staff and providers, patients, and families. Essential Functions Knowledgeable about services for Obtains up to date knowledge on relevant developments and public policies that affect refugees. Creates/obtains useful tools and guides about such services for staff and refugee use. Coordinate with external refugee services, creating partnerships to help identify and recruit refugees to OCH clinics. Provide health and mental health navigation by assisting patients with services such as, but not limited to, health insurance enrollment, registration, provider identification, appointment scheduling, management of clinical costs, appointment reminders, form completion/form submission, and transportation. Communicates/coordinates with care team members. Works with our translation partner to get forms translated promptly. Stays up-to-date on health-related community resources and acts as a point of contact, following up on resource needs and troubleshooting patient barriers to accessing services when necessary. Assists with outbound/outreach telephone calls to reduce gaps in care, schedule provider visits as appropriate. Works proactively to improve patient access, offering timely and accessible appointments, monitoring no-show rates, and following up accordingly. Additional Duties Provide Patient Service Representative coverage when necessary. Verify and update patient demographic information and insurance information. Create encounters and utilize account flags and folder notes as needed. Required to participate as a care team member, working collaboratively with all practice staff. Resolve issues in the spirit of cooperation; create solutions that benefit all parties. Communicate appointment delays effectively with patients and providers; apprise patients of wait times, delays, etc. Maintain knowledge of computer system changes/updates, including required training. Must provide accuracy of data entered into computer systems, following set protocols. Follow One Community Health procedures to report and resolve system problems. Participation in community meetings and engagement activities Other duties as assigned. This includes but is not limited to accurate and timely management of scheduling requests; active monitoring of email, EMR in baskets, voice mail, and person-to-person communication devices; rescheduling and canceling patient's appointments as appropriate; monitoring inventory, organization, cleaning of patient care areas, storage of supplies, etc., Education/Experience Required High School Diploma or GED Previous healthcare experience highly preferred 2 years customer service required.
