Job Description: The Clinician will perform a variety of duties as a member of the Patient Centered Medical Home, Primary Care Team. Work will include consultations, briefassessments, brief interventions, care coordination, and individual, family, and group behavioral health interventions with children, adolescents, andadults. The Clinician will work directly with medical providers and the care team to collaboratively address patients’ care and to support optimal medicaland behavioral health. The counselor will engage in and support patient access, high quality care, and overall productivityof the Team. Educational Requirements: Master’s degree in social work, mental health counseling, psychology, or a closely related field, required. Experience Required: Two to four years direct patient care and managed care experience preferred; public sector experience may beapplicable. This experience may be in an internship. Demonstrated knowledge ofcollaboration needed to achieve common objectives of medical/behavioralhealthcare preferred. Public sectorexperience may be applicable. Professional Licensure/Special Skills and Certification Required: Candidate must be eligible for and actively working towards obtaining an LICSW or LMHC, or other similar credentials. Interpersonal Relationships/Organizational Expectations: 1. Commitment to providing health care to families in a medically underserved community. 2. Willingness to be flexible in the development of new methods of health service delivery. 3. Ability to deal with sensitive information in a highly confidential manner. 4. Effective communication skills, both oral and written, with patients, families, visitors,co-workers, health care professionals, funders and other regulatory agencies. 5. Bilingual in English and another language appropriate to the health center’s patientpopulation preferred, but not required. 6. Ability to function independently with minimal supervision, and to plan, coordinate,and carry out those tasks necessary to meet the job responsibilities. 7. Awareness of resources available within the health center to accommodate the needs ofpatients. 8. Commitment to providing cost conscious, fiscally responsible, timely, efficient,effective, safe, patient-centered, culturally competent, equitable, andparticipatory health care. 9. Processes personal health information in accordance with FHCW HIPPA policies andprocedures. Reporting Relationship: Reports directly to the Manager of Integrated Behavioral Health Powered by JazzHR
