Description:
Looking for a remote Healthcare Behavioral Health Coordinator residing within the states of Indiana to be responsible for health care management and coordination of healthcare members. Schedule is M-F, 8am-5pm EST. Behavioral Healthcare Coordinator Experience in behavioral settings/with the service continuum Experience & knowledge in community resources available Electronic medical record use/documentation Care/treatment plan development Experience with assessments like PHQ9, GAD7, Vanderbilt Engaging challenging populations Essentials Managing tasks in personal queue in TruCare Working through numerous different spreadsheets and creating referrals Outreaching to members to assist them with identified care coordination needs Documenting all interaction with members/providers/other team members Calling providers to assist with appointments, prescriptions, other health related needs Checking and responding to email throughout the day Meet Care Management Department Targets as defined by leadership: Active Case load Expectations: 120 Case Load LOS: 90 days Maintain up to date TruCare Tasks: No tasks overdue greater than 7 days Enrollments per Month: Average of 40 new enrollments per month Call Volume to Meet Outreach/Engagement/Enrollment Expectations: Average of 300 calls per month Meet department audit standards. Audit Standards include: Maintaining an average of 90% on audits completed for the calendar year Quality documentation- documentation is thorough, easy to read/follow, etc. Quality care plans- elements are related to each other, member focused, etc. Appropriate referrals to resources to support member needs Achieve HBR initiatives and HEDIS measure improvement as defined by department leadership. EducationBachelors Degree in relevant field (Social Work, Psychology, or similar field) PME is an equal opportunity employer.
