SERVICE COORDINATORSCurrently seeking Service Coordinators for our Fresno, Merced & Visalia offices providing case management services for individuals with developmental disabilities. Requires BA/BS degree in social work, human services/ behavorial science related degrees + 2 years of experience, OR a Master's Degree in social work, human services/ behavioral science related degree and 1 year of experience. POSITION: Service Coordinator POSITION DESCRIPTION: Coordinate services for individuals with developmental disabilities and children at-risk; advocacy; assistance with problem solving and crisis management; assure consumers receive needed services and referrals to appropriate resources. Must meet state/federal timelines in completion of reports/documentation and meeting eligibility criteria. QUALIFICATIONS: Requires Bachelor’s degree in social work, psychology, human services, nursing, child development, or equivalent degree in human services + two (2) years of experience in case management or working with individuals with developmental disabilities or related field; OR Master’s degree in social work, psychology, human services, nursing or equivalent degree + one (1) year of experience in case management or working with individuals with developmental disabilities. Requires demonstrated computer knowledge for completion of caseload documentation/data entry into data base and e-mail. Automobile and valid driver’s license required. Must demonstrate cultural awareness and sensitivity and an ability to work effectively with culturally diverse populations. Bilingual candidates are strongly encouraged to apply. WORKING CONDITIONS/PHYSICAL REQUIREMENTS : Professional office environment. Requires travel to individuals' homes, community agencies, schools, care provider facilities, appointments and public meetings. Extensive telephone use. Verbal and written communication skills to complete required reporting and communication. Availability and capability to respond to individuals' special needs, incidents and crisis intervention. STARTING SALARY: $22.50 per hour BENEFITS: Accrual of vacation and sick leave hours, holiday pay, group health insurance and prescription plan, dental insurance, vision service plan, life insurance, CalPERS retirement and long-term disability insurance.
