Description:
The Care Retention Coordinator provides operational support to all activities directed towards increasing care retention within the Care New England and Integra networks. This position will produce activity reports for a variety of audiences and escalate issues to leadership as appropriate.Bachelors’ degree with experience in physician operations, provider relations or network development in a health care or managed care setting preferred, or equivalent combination of education and experience. Strong working knowledge of data management, and should possess demonstrated EHR proficiency. Requires the ability to multi-task and strong attention to detail. Deadline conscious and results-oriented. Understanding and familiarity of managed care organizations or accountable care organizations preferred. Excellent communication, presentation, and interpersonal skills required. Must be fluent in the English language and have a valid driver’s license, current insurance and access to a vehicle. Track referrals from PCPs to focused specialties on a daily/weekly/monthly basis Audit “place and provider” of any diagnostic testing or procedures performed or directed by specialists. Responsible for tracking referrals from PCPs to specialists weekly, then interfacing directly with specialists to ensure that procedures and services are provided within network. Escalate issues to hospital leadership, and ACO as appropriate. Monitor and report any obstacles providers may experience in either providing service at in-network hospitals or directing patients to in-network services. Maintains provider tiering database for EHR systems throughout the system. Responsible for creating weekly and monthly reports for various audiences including ACO Network Manager Committee and the care retention committee. Acts as a project manager for care retention projects as necessary. Collaborate with referral hub staff. Maintains professional growth and development. Preserves and protects employee, patient and hospital confidential information. Performs other job-related duties as required or requested. recblid oa3qcroq65tn35sott842b1o3r0p9e
