Job Description: Responsible for assessment of all psychosocial factors and participation in the treatment planning of patient's families. EducationMaster's DegreeExperienceOne year of related experience. Requires working knowledge of standard practices and procedures.Licenses & Certifications*Driver's License *Vehicle Insurance *BLS or must be obtained within 30 days of hire.Qualifications*Degree in social work and currently licensed or pursuing licensure as a social worker in the state of Georgia preferred. *Master's degree preferred from a field related to social work with one (1) year of social work experience acceptable. *One (1) year of home health experience preferred.
