First Choice Health Centers (FCHC) is a Federally Qualified Health Center (FQHC) seeking a passionate Behavioral Health Clinician, LCSW with an opportunity in East Hartford, CT! Our ideal candidate would have a strong drive to work with underserved communities, being a part of an interdisciplinary care team, and have a desire to be a part of a growing team to innovate the care delivery model. Provides direct clinical social work services, including individual, group, and family therapy. Performs case management services. Provides extensive documentation and care coordination. Participates in the Center's quality assurance, performance improvement and risk management activities as required by Center policies and procedures or as assigned by Center leadership. Qualifications Education: Master's Degree from an accredited school of Social Work Professional Certification: Licensed to practice clinical social work in CT Experience: Experience working with a mental health and/or chemical dependency population. Knowledge of brief treatment and Patient Centered Medical Homes (PCMH). COVID-19 considerations: Employees of First Choice Health Centers must be vaccinated against COVID-19.
