The CTL supports individuals who wish to transition from a Nursing Facility into the community. The CTL maintains a regular presence on site at GLSS area Nursing Facilities to assess/screen patients for eligibility for various LTSS and community supports to facilitate community living. The CTL works closely with the Community Transitions Program Manager, the CTLP Program Assistant, and the CAE screening nurse, as well as partnering with nursing facility staff to support discharges to the community. As a member of the interdisciplinary GLSS Home Care team, the CTL actively collaborates with the team to ensure the provision of person-centered services to older adults, people with disabilities, and their caregivers according to the Mission, Vision, and values of GLSS. Position Responsibilities: Acts as onsite point of contact for resident's families, health care proxy (HCP) and Nursing Facilities (NF) staff related to transitions from NF to community. Visits with residents to increase awareness of service and introduce transition to the community as a potential option. Participates in resident family and/or HCP conversations to inform options and transition planning. Facilitates person-centered planning and needs assessment. Completes referrals to other programs and follows-up on referrals to ensure timely transition. Liaises with nursing and other professionals at the Nursing Facility. Ensures that the CTLP team maintains a weekly presence at each nursing facility. Completes Random Moment in Time Study (RMTS) as required by EOEA. Qualifications: Bachelor's degree in social work, human services, nursing, psychology, sociology or a related field OR a bachelor's degree in another discipline and demonstrates experience and/or strong interest in the field of human services via previous employment, internship, volunteer activity and/or additional studies Candidates who have completed at least 50% of the coursework required for a bachelor's degree in an appropriate discipline shall be eligible for employment without a waiver contingent upon an agreement to complete the degree within a timeframe not to exceed three years. Must possess knowledge of long-term care, case management, discharge planning, community resources, programs, and benefits to help support an individual's transition from an institutional to a community setting Must demonstrate a passion for supporting consumers in achieving their personal goals related to wellness and community living Ability to make proper judgments, good written and verbal skills, demonstrated leadership qualities (for supervisors/mgrs.) and ability to maintain effective interpersonal relationships with staff members, consumers and partners Possess the knowledge and skills necessary to work successfully in the interdisciplinary team and to provide or arrange for services Proficient in the use of computer systems, including but not limited to, Windows based software application including but not limited to Word, Excel, PPT and Outlook Ability to process information accurately Ability to work both independently and as part of a team Professional demeanor and ability to handle confidential materials with discretion Must have reliable transportation and valid driver's license in good standing Bilingual skills preferred Hours 40 hours per week, Monday through Friday *GLSS is an affirmative action and equal opportunity employer.
