Title: Resource Coordinator (Inpatient) Description: This position is an entry level position well-matched for individuals interested in pursuing a career in the social services field. A high degree of interpersonal, organizational and critical thinking skills are necessary for this position. The Resource Coordinators works with the team to identify and secure community services and aftercare resources for individuals upon discharge from inpatient psychiatric care. The Resource Coordinator is an active and valued member of the care team for our adult and child/adolescent inpatient psychiatric units. Qualifications : Bachelor's degree in the human services field AND 1-year related experience - preferable in case management Responsibilities: Coordinates discharge planning with other members of the multidisciplinary treatment team. Works collaboratively with team members to secure a comprehensive discharge plan to meet the biopsychosocial needs of individuals in care. Maintains knowledge of programs and community resources Develop and maintain positive working relationships with community agencies and advocates for the needs of individuals in care.
