The New Graduate Home Health Nurses Residency is an exciting opportunity for motivated individuals who are passionate about community-based nursing. A DAY IN THE LIFE OF A NEW HOME HEALTH REGISTERED NURSE (RN) The Residency Program is a transition to practice program. You will p rovide professional nursing care to patients in varying states of health and illness. Working closely with the preceptor during the initial orientation period, the Nurse functions as an essential member of nursing and multidisciplinary teams providing direct care to patients and families, teaching, and preparation for continuing care after discharge. Skills you will learn as a Home Care RN: Actively manage the patient's treatment and needs. Accurate documentation of nursing care provided to ensure the effective integration of information. Take action to ensure that infection control standards are maintained and stay educated on a variety of high-tech skills, including IV use and maintenance, wound VAC, and complex wound care. Carry out visits with patients in a timely manner and ensure that patients and caregivers are educated on health matters. Coordinate with a wide variety of physicians, providers, team members, and vendors in order to fulfill the recommended plan of care. Assist with any patient discharges or transfers and complete all the correct paperwork and reports. QUALIFICATIONS Graduate of an accredited school of nursing, BSN preferred. State of Vermont Registered Nurse (RN) license required or compact licensure recognized by the State of Vermont required, or a graduate nurse who is license-eligible for the first time. Valid state driver's license and vehicle required. Ability to successfully pass motor vehicle record check, child abuse, criminal record check and adult abuse check required. ADDITIONAL INFORMATION The start date of this program is September 11th, 2023. NCLEX- RN must be passed prior to the start of the program. This is a paid, full-time, 12 week training program followed by a 3 to 6 month residency. This is a full time, plus call, position once orientation is complete. No per diem or part time hours will be granted. VNA & Hospice of the Southwest Region earns a competitive wage .
