Licensed Practical Nurse - Direct Care Responsibilities: Participates in the planning, implementation, and evaluation of nursing care. Direct care training of person(s) with developmental disabilities in the residential treatment setting. ESSENTIAL FUNCTIONS: Provides direct and individualized nursing care, including medication/treatment administration, to an assigned client, under the supervision an RN Prepares, administers, and records medications in accordance with the policy regarding the administration of medications by a licensed practical nurse. Monitoring and review or MARS, medication storage and documentation. Assist in obtaining informed consent for medication and/or treatment Keeping emergency sheets updated and accurate. Shares responsibility for the planning, implementation, and evaluation of nursing care plans Participates in client's team meetings, sharing knowledge, information, and ideas among team members Ensuring physician orders are current, properly documented and communicated to direct care staff Working directly with physicians and specialists to plan medical treatment Ensuring medical appointments have been kept and information communicated to agency staff Routine monitoring (data collection) of specific medical symptoms Training/education of direct care staff regarding client's medical issues. Participates in emergency care when such need arises Records information concisely, accurately, and completely implements by ''hands on" programs and activities as deemed priority goals by the interdisciplinary tea Follows and implements behavior management procedures and plan Interacts frequently and positively with residents using appropriate role modeling encouraging proper conduct, manners and appearan Observes and responds to signs of illness, injury and ill effects of medication. Kl'OWLEDGE, SKILLS, AND ABILITIES:Demonstrated clinical competencyAssumes responsibility for own continued personal and professional growth.Ability to work cooperatively and communicate effectivelyEDUCATION AND EXPERIENCE:Completion of a formal education program in practical nursingCurrent state licensureCompletion of additional training relevant to specific client needs and issues.
