North Country HealthCare is transforming healthcare for rural Arizona as a Federally Qualified Health Center and Teaching Health Center. We provide Primary Care, Dental, Physical Therapy, and Behavioral Health services to the under-served population across 12 communities in northern Arizona. We have an interprofessional care practice in a culture that values innovation . The Ryan White Medical Case Manager and Health Educator is a full-time position for the HIV/AIDS Ryan White Part B Program and the SABG Screening & Education Program . This hybrid position under the Outreach Department at North Country HealthCare will provide HIV and HCV screening and education in clinic-based and behavioral health classroom-style settings for individuals with substance use disorder in Coconino and Yavapai counties. Additionally, as a Medical Case Manager they will work to coordinate the medical care of patients living with HIV/AIDS in northern Arizona. Coordinating care involves processing eligibility for Ryan White and linking clients to services such as dental, primary care, HIV specialty care, behavioral health, and support services. Case Management is provided both onsite at North Country, at partner clinics, and in client home visits. The position requires strong verbal and written communication skills, attention to detail and organizational skills, the ability to think critically to solve problems and to make independent decisions. Computer competency including Microsoft Office programs and the ability to learn to work in different databases is required. The ideal candidate has an understanding of the populations served, barriers to care; disease prevention and transmission, and sensitivity to the social stigma that exists for persons living with HIV and those struggling with substance use. Candidates should be comfortable discussing sexual safety, substance use and harm reduction. Treating all clients in an atmosphere of dignity and respect is essential. Travel, sometimes overnight, is required for the position and a valid driver license and safe driving record are necessary. Because travel days tend to be extended hours, it is possible to structure this position in four 10-hour shifts. North Country Healthcare is a medical clinic, and health and safety precautions are taken seriously.
