The ideal candidate would be r esponsible for providing person-centered, strength-based services to adult individuals served within Crisis Stabilization Residential Services and Transitional Services. Assist and support individuals served with stabilizing and reintegrating back into the community after experiencing a mental health crisis. S chedule Options: Full time - Evening Shift Hours - 2 Week Rotating Schedule 2pm - 10pm Job Summary : Responsible for providing crisis intervention, person-centered, strength-based services to adult Individuals served within Crisis Stabilization Residential Services and Transitional Services. Assist and support individuals served with stabilizing and reintegrating back into their preferred community following a mental health crisis. Essential Job Functions: Responsible for Individuals served and operational standards to include, but not limited to: Being the point of contact for referral inquiries and conduct pre-screening tasks to ensure appropriate forms and information are obtained prior to admission. Ensuring referrals, accepted for services, qualify for admission. Obtaining appropriate forms and information upon admission within the required time frames. Providing orientation for new Individuals to the crisis facility. Addressing mental health crisis needs of individuals upon admission and throughout their stay. Assessing the safety of Individuals served including screenings, monitoring, crisis planning, and connecting individuals to appropriate emergency services as the need arises. Informing Transition Services Coordinator and Director of Mental Health Services of each new Individual admitted for CSRS and/or Transitional services. Ongoing communication with provider(s), families, physician, funders etc. while services are being provided. Scheduling Individuals served with a qualified Mental Health Professional daily while in CSRS. Verifying an assessment is completed within 24 hours of admission to CSRS. Contacting Individual's natural supports and providers within 24 hours of admission to CSRS. Working on Individual's goals on daily and documenting progress or continued need for service. Developing and distributing Treatment Discharge Summaries to the required parties timely, upon discharge. Scheduling and monitoring the achievement of CSRS follow-up appointments and documenting all communication within the Individuals' charts with full details of the appointment(s). Checking Medicaid status, request prior authorizations, and request extended stay authorizations as identified. Offering skill building and therapeutic programming daily to reach the required one hour per day of service. Requirements: High school diploma (or equivalency) with a minimum of two years of experience in behavioral or mental health services. Associates/Bachelor's degree in a non-human services-related field is preferred but not required Must complete an additional 30 hours of in-service crisis and mental health training within 180 days of hire. Must have a valid driver's license, a good driving record and show proof of auto insurance. Satisfactory results on background check. APPLY TODAY! We are an equal opportunity employer.
