Community Health Worker (CHW) Dr. Arenia C. Mallory Community Health Center, Inc. (Mallory CHC) is actively recruiting for a Community Health Worker (CHW) . The Community Health Worker is to serve as a liaison/linkage between the patient and the healthcare provider to facilitate continued care, management and prevention of the chronic diseases such as Diabetes, Hypertension, Hyperlipidemia, and HIV. The CHW will serve as a capacity builder to increase the community's health awareness through outreach activities related to advocacy, health promotion, and prevention, and to provide informal self-management health education. RESPONSIBILITIES: 1. Assist in meeting the patient's basic healthcare needs in relation to self-management, advocacy, health promotion, and prevention. 2. Assist with appointments and remind patients to keep follow-up appointments. 3. Assist patient in understanding health information provided by the healthcare provider. 4. Serve as a link between the community and healthcare system. 5. Gauge patient's level of knowledge and understanding regarding medications and chronic disease(s). 6. Encourage compliance and adherence of medication regimen and treatment plan. 7. Offer suggestions for medication compliance. 8. Discuss current lifestyle behaviors and provide informal education regarding lifestyle modifications and prevention. 9. Conduct follow-up phone calls, home visits, and/or group meetings to evaluate progress, compliance, problem solving and self-management, [post office visit, within one month, and quarterly thereafter and/or when needed]. 10. Document home visits, telephone calls, referrals, and follow up discussions. 11. Conduct blood pressure monitoring, measure height, weight, and waist circumference. 12. Demonstrate the proper technique of performing self-glucose monitoring. 13. Assist patient with keeping a blood pressure and glucose monitoring log. 14. Promote HIV prevention through PrEP education. 15. Provide monthly reports of activities, case load, and meet with supervisors (Chief Clinical Officer, Director of Enabling Services and PrEP Coordinator) on a regular basis. 16. Attend all required trainings, (i.e. Chronic Disease Self-Management, Diabetes Self- Management, and PrEP education). 17. Work collaboratively with the Enabling Services Director, Clinical Care Liaison and PrEP coordinator to promote the healthy lifestyles in the community and clinical settings QUALIFICATIONS: Bachelor's degree or 100 college hours in a related field, e.g. sociology, social work, special education, psychology, guidance and counseling 2 years' experience and/or training in a health care setting Must be mature, well organized and able to work effectively and confidently Provider Life Support (BLS) Certification is Required Background Check Is Required TB Skin AND Drug Test Is Required Must Be Able AND Willing to Travel, if needed to Canton, Durant, Greenwood, Lexington and/ or Tchula BENEFITS PACKAGE INCLUDES: Health (Medical) Insurance Dental Insurance Vision Insurance Basic Life and AD&D Insurance Ancillary Benefits (Accident, Cancer, Disability, Critical Illness, Whole Life, Term Life, and Hospital Confinement) Holiday Pay Vacation and Sick Leave Package Retirement Plan (403b) Professional Liability Insurance Coverage Student Loan Forgiveness Program Incentive Production Bonus Paid Professional Membership Fee Allowance Continuing Education Leave and Training Education Allowance Guaranteed Work Schedule: Mon, Tues, Thurs 8am to 6pm and Wed 8am-7pm In addition, your compensation package includes the following - Health insurance paid up to 85%, 403B Retirement Plan, Free Basic Life and AD&D Insurance, MCHC Employee Patient Privileges, Free Employee Assistance Program: "WorkLife Matter" Access (Telehealth), Legal Assistance Services, and Identity Protective Monitoring Services and more.
