Summary: The Peer Advocate is responsible for providing direct service to clients, and for facilitating the smooth day-to-day operation of the program. The Peer Advocate collaborates as a member of the multidisciplinary team (Treatment Team) to deliver effective treatment. This position supports the RSVP program for Veteran's within Phoenix Rising Recovery. Es sential Duties and Core Competencies: Demonstrate appropriate boundaries with clients Support clients in accessing community resources, specifically focusing integration with the County's Coordinated Entry System for housing (CES), U.S. Department of Veterans Affairs (VA), and other Service Planning Area (SPA) based systems. Provide case management for a caseload of veterans in the HUD-VASH program Coordinates care and serves as advocate on behalf of the veterans Transport and accompany participants to appointments and meetings, housing searches, voucher application/issuance sessions, and any other veteran-centered activity. Assist participants with setting up and sustaining self-help (mutual support) groups and with locating and joining existing groups Acts as reference source for client to legal, community, social service, and administrative bodies as needed; facilitates support groups and client participation Communicate with external organizations to provide additional information and coordinate to ensure the client obtains the necessary resources Assist clients and families with completing forms and documents for services. Demonstrate effective crisis intervention Teach independent living skills effectively Support clients who have discharged with follow up calls Engage in collateral duties as directed by program management as needed. Be aware of potential crisis and manage the program to decrease crisis Identify, prioritize, and resolve problems accurately and efficiently Prepare and facilitate groups as assigned Is present at and oversees client activities Education and Work Experience: Experience with military culture and veteran's issues or experience in recovery from mental health and/or substance abuse issues, or experience in homelessness and reintegration, preferred. Associates degree in social services, education or related field OR two years progressively responsible work-related experience in human service delivery coordination OR satisfactory equivalent combination of experience and training upon screening. Registered or Certified Addiction Counselor with a certifying organization approved by CA DHCS Knowledge and Skills: Knowledge of health care, social service resource systems and self-help intervention strategies. Crisis intervention and conflict resolution skills including use of motivational interviewing, harm reduction approach, and trauma-informed care. Knowledge of and belief in the principles of client centered treatment. Knowledge of medications and interventions preferred. Demonstrated ability to work independently and as a team player. Excellent communication skills both oral and written. Able to maintain a professional manner with clients, staff, clients and visitors. Able to work in a fast-paced setting, under pressure and maintain open, and proper communication. Demonstrate professionalism and strong boundaries with the clients. Must be fluent in speaking, reading, and writing English. Benefits: Renewal Health group, offers competitive salaries for all positions.
