The SEES meets with all individuals to assess work/school interests, assists in identifying and selecting options for school or work, and arranges access to benefits counseling as needed. The SEES spends 65% or more of scheduled work hours in the community developing jobs and connections to local education institutions. DUTIES AND RESPONSIBILITIES: Engaging participants and establishing trusting, collaborative relationships directed toward the goal of education or competitive employment in community job settings. Collaborate with the team to deliver SEE support within the context of the model (e.g. engaging with participants to do fun activities, practicing cognitive or social skills to help them get a job). Based on the participant's preferences, working closely with family members, or loved ones to educate them on how to support the young person with work/school goals. Providing job development and job search services directed toward positions that are individualized to the interests and uniqueness of the people on their situation and the situation of their support system, following the principles and procedures of the individual placement and support approach to supported employment. Conducting a minimum of four (IPS is 6) employer contacts per week. Provide support to the participant and the family for education goals in the community, including contact with staff at high schools, colleges, and other educational institutions. Providing individualized support to assist participants in maintaining employment or education. Work directly with other team members to provide support and assistance to program participants. Helping family or loved ones navigate employment and education systems, identify resources that could be helpful to them. Linking transitioning participants to services that can support work and school goals post-discharge from OnTrackNY. Arranging access to entitlements counseling for the individual or family. Assist in the development of community support systems and networks. Provides the opportunity for the youth to expand the skills and strategies necessary to move forward in meeting their personal, individualized life goals, develop self-advocacy skills, and to support their transition into adulthood. Offer support necessary to ensure engagement and active participation of the youth in the treatment planning process and the ongoing implementation and reinforcement of skills. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress Complete QI Training during onboarding and participate in QI refresher training as needed. Work with the entire CN Guidance team to apply the Plan-Do-Study-ACT continuous quality improvement approach to address systemic, organizational challenges Actively engage in agency wide trainings and work groups as well as OnTrackNY specific trainings as well as team meetings. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress Other duties and assignments as requested by the Team Leader EDUCATION AND EXPERIENCE: At a minimum, a Bachelor's degree in mental health related field. Experience providing employment or education services preferred. Training and experience working with adolescents/ young adults with serious mental illness preferred.
