Support Coordinator InReach is looking to hire a Full-Time Support Coordinator. We have an exciting opportunity to join our team as a Support Coordinator! Our Full-time Support Coordinator (QP) will be responsible for developing and coordinating Plans of Care for individuals with Intellectual/Developmental Disabilities. Duties include meeting with customers and families to address needs and concerns; complete all documentation in a timely manner; provide supervision to Direct Care Staff including review of staff documentation and work strategies, provide one-on-one supervision as needed; implementation and review of customers Plan of Care. Must also be willing to work cooperatively with community partners as well as staff from other InReach departments, respond to all needs and requests in a positive and professional manner, be available to provide on-call service for emergencies. Schedule: Monday - Mon Fri between 8:30am 4:30pm, hours may vary depending on customer needs. Night and weekend hours may be necessary. Pay Rate: $47,500 per year This position is partially remote and in-office. Office location is 4530 Park Road Suite 300, Charlotte, NC 28209. Night and weekend hours may be necessary. Responsibilities: Supervising Staff in our Community Outreach Department in the implementation of supports to individuals served. Monitoring, managing, and facilitating operations on a day-to-day basis. Submit and monitor authorizations for services with MCOs Responsible to complete all documentation in a timely manner. Implementation and review of customers Plan of Care. Plan and oversee work schedule, approve payroll hours. Provide one-on-one supervision as needed. Share on-call service for emergencies. Must have access to a vehicle 24-hours a day and be willing to drive anywhere in Mecklenburg Co. Requirements: Bachelor's degree or higher. All Program Managers must have Qualified Professional status; (4-year college degree in human service field plus two-year's supervised habilitative experience working with special needs population, Medicaid Waiver experience preferred. Must have 2 years of supervisory experience. Possess a valid driver's license, have use of a vehicle 24-hours a day and maintain safe driving record.
