Programs IN-HOME THERAPY SERVICES (IHT) under 21 years of age IHT services are delivered by one or more members of a team consisting of Therapist and para-professional (TT&S) staff, offering a combination of medically necessary In-Home Therapy and Therapeutic Training and Support. In-Home Therapy is a structured, consistent, strength-based, therapeutic relationship between a clinician and the youth and family for the purpose of treating the youth's behavioral health needs, including improving the family's ability to provide effective support for the youth to promote his/her healthy functioning within the family. Interventions are designed to enhance and improve the family's capacity to improve the youth's functioning in the home and community and may prevent the need for the youth's admission to an inpatient hospital, psychiatric residential treatment facility or another treatment setting. OUTPATIENT SERVICES (OP) Child, Adolescent, Adult Outpatient Services are behavioral health services that are rendered in an ambulatory care setting, such as an office, clinic environment, a client's home, or other locations appropriate for psychotherapy or counseling. Services consist of time-effective, defined episodes of care that focus on the restoration, enhancement, and/or maintenance of a Member's optimal level of functioning, and the alleviation or amelioration of significant and debilitating symptoms impacting at least one area of the Member's life domains (e.g., family, social, occupational, educational). The goals, frequency, intensity, and length of treatment vary according to the needs of the Member and the response to treatment. A clear treatment focus, measurable outcomes, and a discharge plan including the identification of realistic discharge criteria are developed as part of the initial assessment and treatment planning process and are evaluated and revised as needed. Job Description IHT Clinician who may work in a team that includes one or more qualified paraprofessionals. Implement treatment plan to assist the youth and family in achieving the goals of that plan. The paraprofessional assists the clinician in implementing the therapeutic objectives of the treatment plan designed to address the youth's mental health, behavioral and emotional needs. This service includes teaching the youth to understand, direct, interpret, manage, and control feelings and emotional responses to situations and to assist the family to address the youth's emotional and mental health needs. Phone contact and consultation are provided as part of the intervention. Outpatient Clinician provides intensive clinical therapeutic services to children adolescents, and adults with identified mental health or substance abuse diagnosis. This service is designed to meet the specific mental health needs of the identified client as they relate to their diagnosis and can be provided in-home, in-office, in-school or other approved/appropriate community setting. Outpatient therapy includes psycho-educational activities/interventions designed to assist the client of the reduction of dysfunction. Services are provided with client-centered and family-centered approaches and are delivered through intensive, time-limited, goal specific interventions. Therapist Requirements: Master's Degree in relevant field with post-degree experience working with youth with behavioral/mental health challenges and supervising colleagues in a clinical setting. The Therapist will provide mental health assessment and treatment services to children, adults, and families in home-based or outreach programs. Ensure compliance and maintain standards of care. Attend staff meetings and training meetings as required and to regularly meet with the supervisor to carry out supervisory directives to ensure quality and efficient practice. A comprehensive home-based, behavioral health assessment inclusive of the age appropriate version of the Massachusetts CANS that occurs in the youth's home or another location of the family's choice. Development of a youth- and family-centered treatment plan by the qualified clinician in collaboration with the youth, parent(s)/guardian(s)/caregiver(s), and with required consent, in consultation with other providers. Review and modification of the treatment plan as necessary. Review/update or development of a Safety Plan and/or other Crisis Planning Tools (Safety Plan, Advance Communication to Treatment Providers, Supplements to Advance Communication and Safety Plan, Companion Guide for Providers on the Crisis Planning Tools for Families) in collaboration with the youth and parent/guardian/caregiver. Intensive Family Therapy that may include working with the entire family, or a subset of the family, to implement focused, structural, strategic, or behavioral techniques, or evidence-based interventions to enhance problem-solving, limit-setting, risk management/safety planning, communication, skill-building to strengthen the family, and to advance therapeutic goals or improve ineffective patterns of interaction Initiate contact with new referrals and/or referral sources in a timely manner - at least according to program standard. Review and evaluate the performance of the therapeutic treatment and support staff. Ensure the proper implementation of treatment plan. Meet with collateral service providers to coordinate care across all settings Demonstrate implementation of treatment plan as needed. Prepare and submit all required documentation in a timely manner. Write plans/reports as required. Execute case openings according to program standards and guidelines. Visit in-home treatment clients according to program guidelines. Provide billing data to biller for individual clients and clients with unusual billing requirements. Attend all referral source meetings as indicated. Interface with referral sources, clients, and families to provide program information as appropriate Schedule and attend all supervision as required with Clinical Director Ensure that supervision documentation occurs and is submitted for filing.
