CPC is seeking candidates for our Resource Navigator position. Benefits include... New hire incentive payment of one thousand five hundred dollars, paid in two installments. Eligibility for health, paid dental, and paid vision coverage. 401(k) plan, with 3% match. Paid time off, sick time, and vacation time. 10 company-paid holidays plus 2 floating holidays. Tuition Reimbursement. Pay differential for bilingual staff. Summary : As the first point of contact for clients, p rovides individualized, culturally sensitive support to the whole family using a relationship-based and client-centered approach to coordinate services internally and externally. ESSENTIAL FUNCTIONS Manage walk-in clients, phone calls, and scheduled appointments efficiently and in conjunction with team members. Offer customer service to internal and external clients. Works with diverse individuals while demonstrating good communication skills both oral and written. Develop and maintain relationships with community agencies and groups to coordinate referrals and services to clients; help to identify gaps in services. Interview and assess client's needs to determine program placement and potential referrals. Follow-up on referrals to ensure client needs are met. Complete timely and accurate intake, maintain accurate records, and adhere to confidentiality standards. Advocate on behalf of the client to ensure timely, effective, and appropriate delivery of services. Provide timely and accurate reports to meet internal and external requirements. EDUCATION AND OTHER REQUIREMENTS Associate degree in social services or a related field; plus at least 2 years of related experience as a crisis and intake worker is preferred. At least 2 years of experience in case management. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the public. Experience with Microsoft Office Suite in a Windows environment is helpful and preferred. EOE This position requires proof of COVID vaccination or approved exemption.
