Description:
The Direct Care Worker provides direct assistance and supervision to adults living with physical disabilities with activities of daily living such as homemaking, personal care, community, and personal development skills in accordance with the person's individual service plan. The Direct Care Worker ensures the consumer receives services, in addition to, supporting and promoting their rights, choices, self-determination, independence and build personal and community living skills. Duties and responsibilities /Metrics To Provide effective services and supports for consumers, the essential functions include, but are not limited to the following: Ensure complete confidentiality of all consumer medical information in compliance with relevant legislation and agency policy and respect for cultural differences Develop positive rapport to support the consumer to and/or provide homemaking services such as house cleaning, meal preparation and assistance, laundry, grocery shopping, and banking. Provide direct personal care to consumers based on their needs. Reinforce teaching and support consumers to develop daily living skills using assistive devices and techniques on a daily basis based on their unique needs. Assist consumers to achieve goals as identified in their individual service plan. Assist in the implementation of the consumer's daily living activities and schedules. Implement established protocols and intervention strategies including behavior management. Advocate for consumers based on goals, needs, and individual service plans. Implement a variety of communication strategies to assist the consumer to understand their rights and responsibilities. Assist consumers in social, leisure, educational, recreational, and/or vocational activities as required. Communicate with other agencies, staff, professionals, and family/friends as required to assist consumers to achieve goals. Communicate any significant changes in the consumer's physical, emotional, or spiritual well being to other members of the support team. Participate and ensure a positive team environment Maintain relevant documentation as required or mandatory agency training, meetings. Understand and comply with all agency policies and procedures. Perform other duties as assigned. Qualifications CNA, HHA Preferred or 1-year equivalent experience Experience: HS Diploma or GED 18 years of age or older Valid State ID Background checks Pass TB Testing Previous experience is preferred Cooking and household skills necessary Ability to demonstrate initiative and flexibility to support consumers.. Ability to resolve conflicts and solve problems in a professional manner. Ability to work independently and within a team environment. Ability to advocate on behalf of consumers. Represent the company in a positive and professional manner at all times.
