Guardianship Case Manager Dept./Program: Family Services Division: Family and Social Services Reports To: Family Services Supervisor Supervises: N/A Revision Date: 01/25/2021 Employment Status: Full-time FLSA Status: Non-Exempt Schedule: 40 hours a week Preference: TERO Ordinance 2017-01, P.L. 93-638 Job Summary Under general supervision, the Guardianship Case Manager is responsible for the day-to-day management of client cases in guardianship status with Tribal Court, including coordination with Case Workers, development and monitoring of service plans to ensure the consistent provision of quality services in alignment with client-directed goals and objectives, and facilitation of internal and external resource referrals for legal guardians appointed by the Tribal or other Court Systems. Essential Functions Communicates with community-based service providers to ascertain availability of social, educational, career, housing, and other supportive services on behalf of guardians legally appointed to care for the person/wellbeing of children and/or vulnerable adults. Follow up on referrals. Responds to client, family, and provider calls and concerns while ensuring excellent customer service. Appears in Court proceedings on behalf of guardians as required. Creates and maintains case files for guardians to ensure that physical and electronic case management systems are up to date, including data collection for specific grant and other funding sources as required. Gather information from formal and informal supports. Completes client's yearly assessment and annual reports to the Courts, Social Security, Medicaid, and other authorities and/or benefit agencies as needed. Maintains client confidentiality consistent with professional standards and federal, state, and/or local regulations. Reports changes in a guardian's situation in a timely manner to the appropriate agency/entity to ensure benefits are not jeopardized. Manages workload to ensure deadlines are met. Attends trainings and stays apprised of issues and information necessary to perform work duties. Consistently and effectively utilizes position procedures, recommends changes when necessary, and completes updates as required. Working Conditions Lifting Requirements Sedentary work Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met. Experience Two (2) years' case management experience in family or social services environment. Experience writing reports and creating dashboards. License/Certification Must have a valid Alaska Driver's License and remain insurable under the Tribe's policy. Special Skills Knowledge of Court orders and procedures preferred. Knowledge of common office procedures, equipment, and computer competency, including Microsoft Suite. Skill in establishing and maintaining effective working relations with co-workers, and the public. Excellent communications, both verbal and written, for documenting written reports.
