Role Responsibilities: Administers medications and performs treatments as ordered by physician. Documents appropriately. Assists with direct resident care as necessary, including personal care, feeding, care of environment changes, and safety. Ensures recording of Intakes and Output, toileting records, turning and positioning sheets and other documentation in neighborhood. Completes incident reports and, initiates investigations occurring on assigned unit, and forwards to RCC Coordinator. Notifies RN for assessment as needed. Initiates the transfer of residents to the hospital as necessary and ensures completion of necessary paperwork. Qualified LPN candidates will have: Graduate of a state approved school of nursing A current license as a Licensed Practical Nurse in New York State Ability to work well with others. Genuine interest in the care of geriatric residents Willingness to perform all duties to support Teresian House "Resident-Centered Care" concept CPR certification.
