Team Leader - Florida Assertive Community Treatment (FACT) Community Charlotte Behavioral Health Care (CBHC) is our area's largest not-for-profit provider of community-based behavioral health care, offering a full range of mental health and substance use services in Port Charlotte, Punta Gorda, Englewood, North Port, Sarasota, and DeSoto and Lee Counties. Last year, we served over 11,000 individuals and families. CBHC's mission is to partner with our patients to instill hope, inspire growth, and embrace life. CBHC strives to provide compassionate care to individuals with mental health and substance use disorders through a variety of programs. Our goal is optimal behavioral health and improved quality of life for all children, adolescents, adults, and families. Position Summary The FACT team utilizes a transdisciplinary approach to deliver comprehensive care and promote independent, integrated living for individuals with serious mental health disorders. FACT teams primarily provide services to individuals where they live, work, or other preferred settings, and are available 24 hours a day, 7 days a week. FACT is recovery-oriented, strengths-based, and person-centered. FACT teams provide a comprehensive array of services for program participants, such as: helping find and maintain safe and stable housing; furthering education or gaining employment; education about mental health challenges and treatment options; assisting with overall health care needs; assisting with co-occurring substance use recovery, developing practical life skills; providing medication oversight and support; and working closely with the individual's families and other natural supports. The programmatic goals are to prevent recurrent hospitalization and incarceration, as well as improve community involvement and overall quality of life for program participants. This is achieved through: Implement with fidelity to the Assertive Community Treatment (ACT) model, Promote and incorporate recovery principles into service delivery, Eliminate or lessen the debilitating symptoms of serious mental health and substance use disorders, Meet basic needs and enhance quality of life, Improve socialization and development of natural supports, Support with finding and keeping competitive employment, Reduce hospitalization, Increase days in the community, Collaborate with the criminal justice system to minimize or divert incarcerations, Strengthen parenting skills for those who have children, and Lessen the role of families and significant others in providing care. The FACT team is comprised of a Team Leader, Psychiatrist, Registered Nurse and Licensed Practical Nurse (LPN), Peer Specialist, Substance Use Specialist, Vocational Specialist, Case Manager, and Administrative Assistant. The Team Leader establishes, administers, and directs the program with the following duties: Direct day-to-day clinical operations of the team. Schedules staff work hours and on-calls to assure appropriate coverage. Lead the daily Team Clinical Staffing and treatment planning meetings. Direct and coordinate patient admission process as well as the treatment, rehabilitation, and support services of the program in coordination with the psychiatrist. Functions as a practicing clinician on the team. Conducts assessments and provides direct clinical treatment, rehabilitation, and support services to patients. Directs and coordinates the comprehensive assessment of psychiatric history, mental status, and diagnosis for each patient. Provides clinical supervision, leadership, and service coordination, including on-call and crisis intervention. Completes staff recruitment, interviewing, hiring, and evaluation of staff performance. Compiles and submits monthly and quarterly reports, ensures program compliance with FACT Model and contract, and monitors the disbursement and tracking of program enhancement funds. Minimum Requirements Florida license as a clinical social worker, marriage and family therapist, or mental health counselor, psychiatrist, or psychologist. Will consider a masters level clinician who is eligible for licensure. Three (3) years' experience working with adults with severe and persistent mental illness. One (1) year supervisory experience. Experience with DBT therapy techniques preferred. Ability to pass Level II Background clearance, drug test (CBHC is a drug free workplace) Ability to work independently in a fast-paced environment in collaboration with other community partners. Valid FL driver's license, insurance, and a safe driving record.
