Position Requirements Must be fluent in American Sign Language. Must have a reliable vehicle that can pass a vehicle safety inspection, a valid CA driver's license, auto insurance with policy limits acceptable to VPCI's insurance carrier, and a good driving record demonstrating a history of safe driving habits. Must express desire to work with DHH individuals with I/DD (Deaf Plus). Must be willing to undergo a background check and pre- work drug screening. Must be able to communicate well verbally and in writing. Must be able to lift 50 lbs. Must be effective in carrying out duties while out in the community. Must be able to utilize technology for service delivery and company collaboration. Must complete all items listed in the employee checklist (I-9, W-4, App., etc.) If you are hired, based upon the CDPH Order of September 28, 2021 requiring all of our employees be vaccinated for COVID-19, we will require you to prove that you have received the COVID-19 vaccine or you are entitled to an exemption from vaccination based upon religious or medical reasons. If you are deemed by VPCI to be entitled to an exemption, you will be required to wear a mask while working and you must be tested for COVID-19 on a weekly basis. General Responsibilities: Promote accessibility to services and community for deaf/hard of hearing program participants. Assist with overall development, implementation and supervision of service plans for assigned caseload. Facilitate activities with a person centered approach, monitoring daily needs of those served and encourage positive community engagement. Facilitate/oversee the development and meaningful implementation of Individual Service Plans (ISP). Promote and encourage participation in the person centered programs and services offered. Support individual autonomy, independence, advocacy and self direction through services offered. Assist participants in overcoming barriers to growth/development in all areas of life. Maintain accurate and confidential records on persons served. Work directly with participants, facilitating daily services and ASL communication assistance for individuals who are deaf/hard of hearing. Help keep VPCI competent to serve the needs of deaf plus participants and remain aware and responsive to the deaf culture and community. For referrals of individuals who are deaf/hard of hearing, conduct tours and/or offer interpretation services during tours, orient new persons served to the program and conduct initial strengths & needs assessments. Implement programs within the philosophy of VPCI while demonstrating knowledge and practice of individuals' needs for services and support. Coordinate transportation for access to employment/community opportunities. Establish and maintain positive community support for VPCI and persons served. Chart/document daily on goal progress/development and barriers to outcomes. Develop, implement and monitor Individual Service Plans for deaf/hard of hearing caseload assignments to ensure quality outcomes through service delivery. Assist with preparing deaf/hard of hearing individuals for successful employment outcomes through training, educational opportunities, job coaching supports and evaluation for assistive technologies as needed. Prepare for and lead annual meetings for deaf/hard of hearing participants to track progress, reinforce meaningful services and reduce barriers. Complete quarterly and semi-annual reviews for the same. Maintain professional relationships and communications with persons served, families/caregivers, the public, employers, CVRC and other stakeholders. Communicate regularly with relevant VPCI staff and managers regarding individuals' needs, interests/desires, concerns and behaviors. As needed, offer ASL guidance and instructions to other staff members in support of enhanced services for deaf/ hard of hearing participants. Attend staff trainings and meetings. Lead a zoom virtual hangout for VPCI persons served one day a week to connect with individuals and nurture healthy rapports. Collaborate with other Case Managers to develop and implement ideas/activities for persons served. Perform other job duties as assigned.
