Company Overview The Daughters of Charity is a congregate living residence for the Daughters of Charity. Independent, assisted and memory care is available for the retired Sisters of the Daughters of Charity. Position Summary The Director of Memory Support is responsible for providing overall leadership and management of the memory support, residential department of the Daughters of Charity Ministries. Summary of responsibilities includes promoting excellence in resident care in accordance with each resident's individual care plan, implementing life enrichment programming, providing outreach and support to resident legal guardians, being resourceful and knowledgeable of the evolving information on Alzheimer's Disease and other forms of dementia, assisting in the recruiting, hiring, training of team members to work in memory care, and team member recognition and engagement. Key Responsibilities/Essential Functions Works cooperatively and efficiently with Daughters of Charity Leadership, St. Louise House Administrator, consultants, and staff in all aspects of clinical services. Develops and maintains a program to support and manage the overall daily functions of the memory support department driving continuity and consistency in delivery of high quality services Establishes and maintains open and effective communication with residents, their legal guardian, and team members Works closely with team members in reporting significant clinical changes to the nursing team for appropriate monitoring and follow-up Implements approaches and services to maintain or enhance resident independence Ensures team members provide meaningful activity and engagement opportunities for residents Works collaboratively with the Life Enrichment department to ensure all necessary life enrichment supplies and resources are ordered and available in a timely manner Creates, implements, and maintains onboarding and ongoing dementia training for all team members who work in the memory support department Creates, implements, and maintains onboarding and ongoing life enrichment training that includes how to identify appropriate engagement opportunities for all types of dementia and levels of the disease process Responsible for assessing, planning, implementing, and evaluating the overall needs of each resident in collaboration with the nursing team Other duties as assigned Management Responsibilities Directly supervises or oversees the supervision of the clinical staff and ensuring compliance within the Memory Support Neighborhood in St. Louise House Leads, coaches, develops and evaluates a committed , high performing clinical staff Ensures HIPAA compliance with resident and employee information Promotes/provides educational opportunities for staff Behavioral Skills Possess a personal presence characterized by a sense of honesty, integrity and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals and values of the Daughters of Charity. Possesses a willingness to learn and apply Catholic Healthcare principles and values in the discernment and decision making process for all clinical matters Exhibits well developed verbal and written communication skills and is able to professionally receive and follow oral instructions in English Exhibits safe, positive, and productive work habits Qualifications Education and/or Experience Graduate of an accredited School of Nursing An RN is required for skilled nursing. A BSN from an accredited college or university is desired. Minimum 2 years of experience in a healthcare setting; experience in a setting for seniors with dementia is preferred Unencumbered license to practice as an RN in NYS CPR Certification required Dementia Practitioner Certification (CDP) required
