Description:

DUTIES & RESPONSIBILITIES: Assess health and diet needs for diabetes patients and local community members. Counsel patients on nutrition issues and healthy eating habits. Perform initial and follow-up nutrition assessments, implement interventions and the personalized nutrition plan of care. Review, analyze, and make recommendations to the Senior Program monthly menus. Provides instruction and counsel to patients and/or significant others according to protocol and documents appropriately. Promote better nutrition through public speaking and community outreach program. Document patient progress in the electronic health record. Assist with and prepare reports as required by the grant guidelines. Keep up with the latest nutritional science research. Abide by confidentiality guidelines as required by professional standards. Participate in committees as assigned. Other duties as assigned. REQUIREMENTS: Must possess a Registered Dietician (RD) and/or Registered Dietician Nutritionist (RDN) License in the State of Iowa. Prefer knowledge of diabetes. Must be able to communicate effectively with Meskwaki community members and be sensitive to tribal beliefs and traditions. Must be familiar with HIPAA policies and compliance. Must have good verbal and written communication. Must submit to and successfully complete a background investigation. SUPERVISION: Supervised by the Diabetes Program Coordinator. SALARY LEVEL: Annual Salary Range: $56,000 annually Job Posted by ApplicantPro
