Philadelphia FIGHT Community Health Centers Women and Youth Social Worker Job Description Hours: M-F 9:00 a.m. to 5:00 p.m., Department: Lax Center Supervisor: LAX Women's Program Supervisor Full-Time, 40 hours per week; Philadelphia FIGHT is a Federally Qualified Health Center serving primarily low-income people in Philadelphia. Philadelphia FIGHT Community Health Centers provide culturally competent comprehensive primary care, and state of the art HIV primary care to low-income members of the community, along with research, consumer education, advocacy, social services and outreach to people living with HIV and those who are at high risk, including family members, communities with high rates of HIV, formerly incarcerated persons, and young people at risk; and access to the most advanced clinical research in HIV treatment and prevention. In collaboration with the client and a multi-disciplinary health center team, the Women and Youth Social worker will enhance existing services by providing mental health interventions and addressing the social determinants of health for Women and Youth living with HIV. Medical and community-based social work services may include care coordination, benefits access, treatment adherence, health literacy education, disease and violence prevention education, and counseling for cognitive behavioral skill building and stress reduction. You will serve as an integral part of the multidisciplinary health care team and as the client's advocate in addressing their social work and behavioral health needs. Job Responsibilities Meet with clients unable to attend CBH funded counseling to provide supportive individual counseling Co-facilitate weekly trauma-informed group for Women Living with HIV Ability to be flexible and use non-traditional approaches in working with young adults who are initially difficult to engage in consistent medical and supportive mental health services. Work with Behavioral Health Counselor (BHC) and Medical Case Managers to link clients to appropriate level of care for mental health and substance abuse issues Provide in the moment support and on-call crisis intervention support. Hands on troubleshooting to model de-escalation for staff with Women and Youth Program clients Work on retention efforts to keep clients engaged in medical care Conducts psychosocial assessment on all new clients referred to program. Develop individualized service plan to focus on retention in HIV in care and viral load suppression. Maintain service documentation to include client progress notes, observations, and interactions about clients in patient record (EMR). Develop community linkages. collaborate with the clinical care team Work within a team environment to collaborate on cases and provide feedback on service delivery model. Work closely with both internal and external medical and social service providers to ensure follow up adherence to the treatment plan. Maintain regular communication with clients. Collects and submits data as indicated Perform other related duties as assigned. Knowledge/Skills/Abilities: High level of skill at building relationships and providing excellent customer service. High degree of knowledge and competency in mental health treatment plans. High level of problem-solving skill to better serve patients and staff. Strong attention to detail and accuracy. Excellent verbal and written communication skills. Demonstrated experience conducting mental health services in both single and group settings. Demonstrated experience writing and executing treatment plans. Demonstrated success in collaborating with other healthcare providers regarding treatment plans In addition, the position requires a reasonable combination of skills in the following areas: Strengths based, solution focused, and family centered practice Trauma informed care framework Experience working with diverse client populations, particularly racially or ethnically marginalized groups and the LGBTQ+ community Preferred Skills: Experience working with people living with HIV Experience working with people with substance use disorder Qualifications/Minimum Education: Master's degree in Social Work (MSW). LSW preferred Work Environment: Works in an office environment. Physical Demands Ability to continuously stand, walk, or sit. Ability to bend frequently. Ability to lift up to 10 pounds Ability to squat occasionally Philadelphia FIGHT requires proof of COVID-19 vaccination series for all new employees hired.
