Description of Nursing Staff: As a Home Healtth Nurse, you will assume responsibility and accountability for the application of the nursing process and the delivery of patient care. The Nurse ( LPN/RN) demostrates the ability to make clinical judgements in an effective manner under the direction and supervison of the Nurse Manager. Responsibilites: Utilizes the nursing process to asses, plan, implement and evaluate patient care. Asses signs and symptoms indicating physiologic and psychosocial changes inthe patient's condition Collects, analyzes, and interperts data and information from health care members and documents actual and/or potential nursing diagnoses Documents the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions Performs interventons according to identified priorites, plan of care, and the hospital policies and patient care outcome standard Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ ineffective nursing interventions Uses clinical judgement in evaluation activities to meet patient care neeeds and establishing priorities Home care includes (but no limited to) ventilator care, tracheotomy care, feeding tubes, medication administration, wound care, IV care, pain assesments, oxygen therapies, physical therapy, nutrition needs and proper charting as indicated Other Nurse (RN/LPN) duties as assigned Requirements: Current Nurse (RN/LPN) License for the state of South Carolina Current SCDL and Social Security cards Current PPD or Chest X-Ray Current BLS card SLED check indicating no prior criminal history Negative Drug Screen One year prior Registered/ Licensed Nurse experience preferred Job Posted by ApplicantPro
