The Perm anency Achievement Specialist will be proficient in computer skills in order to complete all documentation required to finish the duties below. Experience working with children with special needs preferred, as well as experience or training in Reunification, Adoption and Su b sidized Guardianship . Responsibilities : Provides consultation regarding permanency issues; identifies and removes barriers to permanency, working to secure appropriate placements for children; monitors and take action to complete permanencies with children waiting for reunification or adoption/subsidized guardianship. Assists the worker with the facilitates of community resources to prepare the home of parent. Assists the worker with post adoption, Guardianship connection to the D epartment of C hild and Family Services unit prior to finalization. Works to secure additional service resources in a timely manner to expedite permanency. When reunification is not possible, assist the worker in listing children for adoption with Greenlight or the applicable Adoption registry. Develop and maintain accurate and confidential documentation, reports, and client records in accordance with Department of Children and Family Service s Rule and Procedure, contracts, and applicable stand ards. Adhere to standards adopted through Council on accreditation. Assists the worker to complete required documentation and home study to begin the Adoption or Sub sidized Guardianship subsidy process. Submits, monitors, and follows up with the needs of the subsidy packet pending D epartment of C hild and Family Services approval. Supports the worker with complet ion o f The Interstate Compact on the Placement of Children doc umen tation to Illinois D epartment of C hild and Family Services needed for out of state permanency options for children with relatives and licenses potential adoptive families, and work with the receiving state to eliminate barriers. Provides linkages between caseworkers, recruitment resource specialists, adoptions staff and community providers to expedite and finalize the permanency process and ensures all necessary actions and reviews have taken place to facilitate positive permanency outcomes. This position does not carry cases. Perform o ther duties as assigned. Requirements : Educational: Bachelor's degree from an accredited university in Social Work or related human services field required . Competency: Basic proficiency in Microsoft Word, Outlook and Excel required. PowerPoint is preferred . Excellent planning and organizational skills. Strong interpersonal and problem-solving skills. Ability to establish and maintain a client focus. Must pass required background check. Ability to set and maintain professional boundaries with clients. Ability to analyze, process, organize and maintain data and confidential information. Must possess valid Illinois Driver's License in good standing and proof of valid vehicle insurance. Utilize time tracking software and all other software required to fulfill duties. Must be CPR and First Aid certified. Ability to be sensitive and understanding of cultural and lifestyle differences of all people regardless of race, religion, sexual orientation, age, or background. Successfully complete FamilyCore training as required. Physical: Frequent sitting lifting, pulling and pushing up to 15 lbs . Regular walking, grasping, fingering , standing and other physical activities related to an office environment. Occasional stooping, reaching overhead. Operate general office equipment, including multi-line phone, fax machine, printer, copier, and personal computer , with or without dual monitors . Work Environment : May be subject to noise, distraction, and interruptions. May be subject to difficult client interactions that involve behavioral and/or emotional challenges. Work occurs in and out of an office environment . Frequently exposed to loud noise and outdoor weather conditions.
