Direct Support Professional (DSP) for Supported Living Program

Description:
Job: DSP needed to provide in-home and community support to consumer enrolled within our unit-based programs: In-Home Community Supports (IHCS) and Community Participation Supports (CPS). Supports include assistance with grocery shopping, personal shopping, household skills, meal prep, assistance with medical appointments along with leisure and wellness activities. DSP will be responsible for electronic documentation of service being provided. Schedule will be 8-4 Monday thru Friday, attending to different consumers in different locations to meet the needs of the program. Expectation: Use of personal vehicle will be needed to transport consumers to appointments and community activities. Vehicle insurance and Registration must be current; copies must be submitted to HR annually or as changes occur. Mileage must be submitted for reimbursement. Be an active member of consumers interdisciplinary team related to ISP development and adherence to services within. Assess consumer needs and communicate with team for optimum support. Complete and submit daily documentation of service provided. Agency computer will be provided. Learn and understand the difference between the two programs, IHCS and CPS, in order to provide allowable services under each program. Be flexible and attentive to consumer needs. Be resourceful in exploring and identifying community locations and events to match consumer interests. Contact supervisor as questions arise. Learn and use the electronic verification system, EVV, while in the IHCS program to accompany electronic billing and service notes.
