We are hiring a Full-time (35 hours/week) CASE MANAGER AT ANGELA'S HOUSE for Catholic Charities in Buffalo 35-hour work week Generous Paid Time Off and Paid Holidays Retirement plan with employer contributions Competitive Salary and Benefit Package: Paid Time Off Medical Dental Vision 403b Retirement JOB SUMMARY: The Case Manager utilizes internal and external resources to assist homeless and/or frail older adults or elderly living in subsidized specialty housing called, "Angela's House". The intent is to help coordinate care and establish stable resources so clients may function at an optimum level of independence through comprehensive assessment, care planning, linkage with essential services and monitoring of service delivery outcomes. The Permanent Supportive Housing (PSH) Case Manager will be a resource regarding services including but not limited to emergency financial relief, housing and eligibility/securing entitlements, health and wellness, caregiver supports, and arranging for in-home nursing services, as needed. ESSENTIAL FUNCTIONS: Exemplify Catholic Charities' mission, vision, guiding principles and policies and procedures Adhere to high ethical standards, consistent with the Catholic Charities' Corporate Compliance Program, Code of Ethics, legal mandates & labor laws, NYS Justice Center, NYS, OMH, managing contractual regulations and other accreditation standards (e.g. BBB). Adhere to all applicable federal and state laws and regulations including, but not limited to those governing confidentiality, privacy, program, billing and documentation standards. Infuse agency values in therapeutic care, promoting quality, client-centered, strengths-based, solution-focused, trauma-informed behavioral health and substance abuse services, as well as with collateral contacts. Cultivate a culture of sensitivity and respect for diversity. Actively participate in required staff meetings and designated training. Perform other duties as assigned. CORE JOB RESPONSIBILITIES: Maintains active caseload of assigned clients residing with Angela's House and ensures all services are in compliance with contract requirements, and accreditation standards; Acts as an advocate for clients; Intake & Assessment: Conduct comprehensive assessments including reviewing referral information, identifying and engaging key participants, identifying individual strengths and needs and the external factors impacting the client; Care Planning & Service linkage: Utilize person-centered practices to assist the client with the development of a person-centered care plan addressing: Activities, services, supports, and providers that will help in recovery, wellness and satisfaction based upon the individual's interests, preferences, and strengths; Skills needed to achieve personal relationships, community inclusion, dignity, and respect; In-Home services that may be required to ensure the client may live independently in their residence. Schedule and facilitate service plan reviews, including evaluation of advancements and barriers to treatment progress; revise the plan as appropriate; Maintain clear and concise documentation of interventions that demonstrate compliance with the principles of interdisciplinary team as well as funder requirements and accreditation standards; EDUCATION: Associates degree in social work, mental health counseling, gerontology or other approved related field preferred. Bachelor's degree preferred. EXPERIENCE, KNOWLEDGE, SKILLS AND ABILITIES: Two (2) years direct experience linking homeless individuals and/or families with essential, community based services. Experience working with frail elderly or adults with activities of daily living challenges a plus. Experience linking those who were previously homeless with resources to maintain housing status. Pleasant, welcoming, and professional demeanor required Demonstrated compassionate behavior toward older adults in need, particularly those who are homeless and/or may have difficulties with certain ADLs Exceptional interpersonal and communication (oral and written) skills required. In-depth familiarity with community health, behavioral health and social services systems, organizations and resources. Ability to increase skills related to interventions necessary to provide effective service Demonstrated ability to engage and maintain effective relationships with a multidisciplinary team Demonstrated agility, initiative, ability to work independently and efficiently (effective organizational and time management skills) and produce results Fluent in various computer applications (e.g. Word, Excel), electronic database systems and electronic health records ADDITIONAL REQUIREMENTS: Requires travel - must have access to reliable transportation, agency-required level of auto insurance coverage, and be able to travel as needed within an assigned service region. Must be prepared to work a flexible schedule in order to participate in agency-related activities/events that may take place outside of typical business hours.
