Responsibilities Assist Community Integration Specialist in providing Habilitation and Home Group (for individuals assigned) services to individuals in Day Habilitation, in providing of consultation and the Habilitation services or other Individualized Service Plan components for which responsible, in assuring compliance with all applicable Day Habilitation Standards. Will work as a team member with Community Integration Specialist and case manager to ensure that Day Habilitation components of the ISP are delivered. It is the responsibility of the team to ensure that all services are individualized and based on choice Qualifications High school diploma or equivalent with six months experience preferable assisting individuals with disabilities, direct experience in a related field or one full year of college education in human services. Must successfully complete all required training including but not limited to First Aid/CPR, SCIP (Strategies for Crisis Intervention and Prevention) and Medication Administration. Must be able to lift, move and transfer up to 35 pounds. Subject to background checks including but not limited to being fingerprinted/criminal history record check, NYS Child Abuse Registry, OPWDD Staff Exclusion List and Mental Health Law 16.34. Must meet LAI Vehicle Operator Requirements. Salary Commensurate with experience. Location Rochester, NY - ICF/DD - East Fork Lake Road Hours 40 hours per week. Schedule Saturday: 11:00 am -9:00 pm Sunday: 11:00 am -9:00 pm Monday: 11:00 am -9:00 pm Friday: 11:00 am -9:00 pm Staff meetings once per month. 0 Job Posted by ApplicantPro
