Salary: $66,289 - $84,158 Grade: 19 The Research Foundation for Mental Hygiene, Inc. is currently seeking a full-time Family Navigator. We seek an organized, detail-oriented person to be responsible for care coordination and follow-up for families of infants with rare genetic conditions that put them at risk of autism spectrum disorder and related neurodevelopmental problems. The Family Navigator will work with research participants/patients in the Prospective Genetic Risk Evaluation and Assessment (PROGRESS) Autism Center of Excellence at Columbia University, in collaboration with the Research Foundation for Mental Hygiene and the NYP Center for Autism and the Developing Brain. They will act as a liaison between research participants/ patients, researchers, and clinical and educational providers, serving to link families of at-risk infants with appropriate services to help with early developmental needs. Background working with individuals with Autism Spectrum Disorder (ASD) and/or other Developmental Disabilities is desired. Case management experience and familiarity with community resources in the New York metro area are also desired. The ideal candidate has the ability to communicate effectively and collaborate within an interdisciplinary team; demonstrated self-awareness to identify learning needs and capacity to apply feedback into practice; flexibility with a calm and professional demeanor; advanced writing skills to generate and review reports; a sense of compassion and active listening abilities; good organizational, problem-solving, and time-management skills. The successful applicant will primarily work at the Columbia University Medical Center Campus and is expected to travel on occasion to the Center for Autism and the Developing Brain (CADB) in White Plains (Westchester), New York, for training or coordination with the CADB Family Navigation team. Duties and Responsibilities: Facilitates care coordination across disciplines (e.g., physical therapy, speech therapy, early childhood education) and provides follow-up for patients/families in the community (e.g., communicates with community-based providers, attends meetings, helps navigate applications for external services, and access external resources). Works closely with families to integrate care across the lifespan and bridge families to services in their communities to ensure their needs are being met both during their participation in our research program from 2 to 36 months of age. Partners with local organizations and service providers to build relationships and connect patients to service providers across settings (e.g., in-home therapists, early childhood providers, educators, and specialty clinics). Facilitates continued maintenance and expansion of resource database, including creating new contacts via outreach calls and in-person visits to learn about new resources for participants/patients. Serves as a representative at community events and activities. Assists with intake/enrollment responsibilities, including completing phone intakes and interfacing with families regarding their clinical services. Performs other related duties as assigned. Minimum Qualifications: Bachelor's Degree or at least 4 years of related experience. Occasional travel required. [ White Plains (Westchester), New York] Preferred Qualifications: Master's degree in nursing, psychology, humanities, social services, education, genetic counseling, human relations, communications, or a related field preferred. At least 1 year of experience in nursing, social work, or similar healthcare background a plus. Bilingual - fluent in Spanish and English. RFMH employees may be required to be vaccinated for COVID-19 depending on work location and must provide proof of vaccination. Individuals that are not vaccinated for COVID-19 are ineligible for appointment to positions that require vaccination. Fully vaccinated is defined as two (2) or more weeks since receiving either the second dose in a two-dose COVID-19 vaccination series (e.g., Pfizer-BioNTech or Moderna) or the first and only dose of a single-dose vaccine (e.g., Johnson & Johnson [J&J]/Janssen).
