JOB SUMMARY: A Resident Care Manager will assist the Director of Nursing in the daily operation and supervision of our Skilled Nursing clinical department. This will include the implementation of Nehalem Valley Care Center policies, as well as cooperation and compliance with state, federal, and local agencies. RESPONSIBILITIES: Meet and assist with new admissions Discuss problems or concerns with residents, family members, staff, and other interdisciplinary care team members A Resident Care Manager, (RN) (LPN) will read the 24-hour report for changes of condition to determine assessments, plans, notifications, and consults needed Review of psychoactive, restraint, and restorative programs Assist with completion of the MDS process Keep residents' family and physician informed of any changes in resident behavior, medications, or conditions of health Complete incident and accident investigation in collaboration with other clinical personnel Other Duties as Assigned REQUIREMENTS: RN (Required) Nursing diploma (associate or bachelor's degree in nursing) Currently licensed/registered in applicable State.
