One Vision is seeking a Full time Outcome Coordinator for our Mason City Team! We are offering a $1500 sign-on bonus! As an Outcome Coordinator your responsibilities include, c oordinating and implementing Individual Support Plans, m onitors and supports progress on goals of the Individual Support Plan to ensure individuals needs are met, e nsures individuals are safe and their personal welfare and rights are supported and respected, and m aintains mandatory compliance with federal, state, national accreditation and One Vision policies, procedures, practices, and guidelines. Qualifications: BA/BS degree in human service-related field/RN license with 1 year of verifiable experience in supporting individuals with disabilities is preferred. High school education/GED with four years of verifiable experience in supporting individuals with disabilities. Certified medication aide or medication manager, if not currently held, must become certified with six months. Valid Driver's license. Proficient computer skills. Mandt training (can be acquired while at One Vision). Schedule: Typically, Days and an on-call rotation. Maybe asked to work in homes to cover 1st, 2nd or 3rd shifts. Come Join our Team!
