Summary: SSVF Healthcare Navigator provides services that include connecting Veterans to VA healthcare benefits or community healthcare services where Veterans are not eligible for VA care. SSVF Healthcare Navigator provides case management & care coordination, health education, interdisciplinary collaboration, coordination & consultation, & administrative duties. SSVF Healthcare Navigator works closely with the Veteran's primary care provider & members of the Veteran's assigned interdisciplinary treatment team, & acts as a liaison between the SSVF grantee & the VA/community medical clinic, & works with a population of Veterans with complex needs who require assistance accessing healthcare services or adhering to healthcare plans. Must successfully pass driver record & background check. Education &/or Experience : Bachelor's degree from four-year college or university; or one to two years related experience &/or training; or equivalent combination of education & experience. Essential Duties and Responsibilities include the following: Conducts assessments of the Veteran in collaboration with the SSVF team, the Veteran, family members, & significant others. Works closely with Veterans to assist them in communicating their preferences in care & personal health-related goals to facilitate shared decision-making of the Veteran's care. Serves as a resource for education & support for Veterans & families & helps identify appropriate & credible resources & support tailored to the needs & desires of the Veteran. Regularly reviews care plan goals with the Veteran, conducts regular non-clinical barrier assessments, & provides resources & referrals needed to support adherence. Periodically evaluates the effectiveness of the resources & referrals provided & makes appropriate modifications to ensure the provision of high-quality care & interventions. Monitors Veteran's progress & maintains comprehensive documentation in the internal ETO Database. Provides comprehensive case management & care coordination across episodes of care, & acts as a health coach. Serves as the subject matter expert on community resources related to the needs of the Veteran. The Healthcare Navigator collaborates with other providers in the ongoing reassessment of the Veteran's healthcare needs. The Healthcare Navigator is responsible for educating the Veteran & caregiver of the available services & assisting them in establishing the appropriate referrals based on the Veteran's preference. Determine the needs, strengths, limitations, & preferences of each Veteran & will engage in problem-solving to identify & reduce barriers to care. The Healthcare Navigator will educate the Veteran & family on the available options for acquiring knowledge & skills for managing health & wellness. Coordinates referrals to VA, community health clinics, & other programs needed to ensure access to healthcare. Follows the care plan to facilitate adherence & collaborates with community providers to maximize the use of VA & community resources. Acts as an advocate for the client, integrating the Veteran's cultural values into their care plan. Assists the Veteran in identifying methods to monitor progress toward meeting health goals & provides ongoing follow up. Consults with other team members & appropriately assesses & addresses the needs of the Veteran. Learn & understand the different roles within the interdisciplinary team & acts within professional boundaries. The Healthcare Navigator will adhere to ethical principles about confidentiality, informed consent, compliance with relevant laws, & agency policies (e.g., critical incident reporting, HIPPA, Duty to Warn).
