Community Health Worker At CHCBH our Community Health Workers s erve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. The Community Health Worker has a close understanding of the community they serve and works collaboratively with providers and other healthcare team staff to provide care coordination and connection to resources and support to program clients toimprove their health and general well-being through education and coordination of care and services.
