Summary : Under the guidance and supervision of the Chief Program Officer or designee, the Community Health Worker Coordinator provides supervision and guidance to Community Health Worker program staff and outreach/engagement services to program participants and the community. Provides overall supervision of community-based activities in the catchment area. Ensures high quality and timely implementation of community-based activities by the CHWs in the catchment area. Builds and ensures strong partnerships with health facilities and other organizations in the catchment area. Ensures safe and accurate use/storage of working kits/tools provided. Maps the location of all CHWs in the catchment area and maintains a register to track those who have received training. Establishes a monthly work plan and calendar of supervision activities. Ensures supervision of each CHW under their responsibility every month. During supervisory visits, supports CHWs in planning their monthly activities. This includes helping the CHW be a connector, an advocate, and a change agent. Help the team identify individuals in the community needing medical, behavioral health, and social services based on program needs. Provides supervision and support to program staff and prepares Performance Appraisals. Ensures compliance with program policies and procedures as required by funders. Prepares and distributes detailed reports required by funders. Attends staff meetings and formal/informal training sessions in/outside the agency. Participates in community activities as a representative of the program when assigned. Is available to work outside regularly scheduled work hours as needed. Help to support the CHW Apprenticeship program. Support Chief Program Officer with the development, coordination, execution, tracking, and reporting of program deliverables. Track and submit documentation for CHW experiential learning and apprenticeship opportunities. Support content development of CHW training and resource, including presentation slide development. Any other duties as may be assigned by the immediate supervisor. Authority: The employee exercises initiative and some independence in establishing his/her schedule for meetings with participants and community agencies. Follows pre-established procedures for program implementation and daily operations. The individual must be resourceful and able to assess situations to determine if a crisis is involved and how to best proceed within the framework of the agency's and the program's policies and procedures. The employee will consult with a supervisor if in doubt before proceeding. Skills and Abilities: Analytical Skills-Ability to use thinking and reasoning to solve problems. Flexible with the ability to work independently and as a team in person and virtually as needed. Ethical-Ability to demonstrate professionalism, conforming to values and accepted standards. Reliability-The trait of being dependable and trustworthy. Communication, Oral-Ability to communicate effectively using the spoken word. Communication, Written-Ability to communicate in writing clearly and concisely. Position Qualifications: Competency Statement(s) Associate degree in human services or related field preferred. Minimal requirement of Hight School or Diploma or GED. At least 2 years of experience working with Latinos, connecting them with community services. Bilingual in Spanish and English. Demonstrated ability to work with other service providers. Basic knowledge of Microsoft Word. Must have reliable transportation and a valid good standing NYS driver's license. An equivalent combination of training and experience other than the specified, if judged to be adequate for the job, may be accepted by the President and CEO.
