Rome Health is currently searching for an RN Health Coordinator to provide medical assessments of CRC patients; develop recommendations based on individual needs. Facilitate referrals as indicated. Review all outside medical information. Provide medical consultations for CRC staff. Assist program director with overall policy and procedure development to maintain compliance with regulatory agencies. Taking and recording measurements of blood pressure, temperature, heart rate, etc. Observing patients under treatment to identify progress, side effects of medications, etc. Administer injections, assist providers; Assist with agency procedures related to Medication Assisted Treatments, and assist with other basic medical care tasks. Obtain pre-authorization for medication when needed and communicate with pharmacies. Collect and witness urine screen samples/oral swabs. Communicate with family or friends to provide advice, comfort, and release instructions. Provide education and information on risk reduction, counseling, and referral services to all patients regarding HIV/AIDS including pre and post-exposure prophylaxis, tuberculosis, viral hepatitis, sexually transmitted diseases, and other communicable diseases. Communicate and participate effectively in a treatment team setting. Assume management duties in the absence of the Program Director and Clinical Supervisor. Perform other duties as requested and observed by the Director. Qualifications: Licenses: NYS RN license RN with 3+ year's experience delivering patient-focused care and developing relationships with fellow nurses, physicians, and patients. Minimum of 2 years experience working with patients in a behavioral health setting
