This is a full time position, with the following schedule: 8:30am-4:30pm, Monday-Friday Position Overview: Reporting to the Director of Social Work, Social Workers at Maria Regina Residence deliver quality social work services to assigned patients, residents and family members. The Social Worker is responsible for coordinating the discharge planning process, participating in interdisciplinary team meetings and quality improvement activities, as well as counseling and meeting the satisfaction of the patients, residents and family members to facilitate adjustments to the nursing home and promote the enhancement of each resident's quality of life. Position Qualifications: The ideal candidate will have a Master's Degree in Social Work as well as active licensure in NYS, with prior experience in a long-term care, rehabilitation or hospital setting. Prior experience working with the elderly population preferred. Additionally, strong applicants will possess strong communication skills, and an ability to work collaboratively across a variety of disciplines and familiarity working with EMR (electronic medical records).
