Administrative Value-Based support: Payer contracting Quality/coding Risk adjustment HCCs Credentialing/billing Population health Clinical VB support: Monthly Educational meeting (Pods) Provider relations 24/7 nurse triage line Case managers Hospitalists Transitional care Social workers Discharge planners Palliative Care Specialists Clinical pharmacists Home health Ancillaries The Opportunity: Premier Family Physicians an affiliate of Millennium Healthcare is growing, and we are looking for a Chief Medical Director for our Texas Market to lead the value-based care initiatives at Premier Family Physicians and Austin Geriatric Specialists Physician Leader! This role will split duties between clinical and leadership, with 40% of the week performing administrative leadership duties and the rest of the week would be spent as a clinician. Administrative duties would consist of items such as:Working with the existing leadership and management structure, to work with Employer's and its affiliates' other physician employees to facilitate the implementation and achievement of Employer's and its affiliates' objectives including, without limitation, the following: Assist senior management in evaluating and setting Employer's overall strategic direction and support the changes required to implement the strategy. Oversee the clinical policies and procedures of the Employer's medical practices. Provide input and direction to the Medical Directors for any applicable ACO or DCE and Managed Care Programs. Provide input and direction to the ACO/DCE Executive Directors. Support implementation of Employer's clinical programs including ACO/DCE, managed care, CCM, PCMH and similar programs to achieve the following objectives: Improve the health of the patient population. Enhance patient experience and outcomes. Work to reduce the per capita cost of care while maintaining the highest quality of care. Enhance the accuracy and completeness of MRA coding. Develop medical management policies and protocols and supporting clinical performance standards. Oversee new program development, including drug therapies, lines of business and revenue. Improve communication and Processes: Provide input and advice on the agenda for the monthly Physician Executive Council meetings. Provide Administration with input and feedback on behalf of practice pods. c. Work with Pod Directors to remove obstacles to productivity and effectiveness. Attend monthly pod meetings and provide input on corporate strategies and tactics. Act as a liaison between the Texas and Florida Physician Executive Counsels. Support Pod Directors in peer counseling and mentoring.
