All employees are encouraged to join and participate in committees, trainings, and workgroups. The Position: We are currently in search of a Primary Care Integrated Behavioral Health Clinician. The Primary Care Integrated Behavioral Health Clinician's main responsibility includes supporting the Population Health Management department in caring for complex patients with behavioral health needs and plays a key role in triaging patient needs identified through screening for social and behavioral determinants of health. This is a salaried, full time position with a schedule of 40 weekly hours, Monday through Friday. This position is located at our Corporate Office in Providence, RI. Employee parking is provided. Essential Functions: Assists primary care providers and care team members in recognizing, treating, and managing mental health and psychosocial issues, acting as a contributing member to the primary care team Consults with primary care providers and care team members in real time to assist patients and families with identified psychosocial issues Triages both scheduled and unscheduled referrals from the medical team, treating some with brief treatment/intervention and referring others to more appropriate resources Provides brief treatment interventions for mild to moderate behavioral health concerns as well as crisis intervention Assists in the identification of "at risk" patients and in the development of plans to prevent escalation of existing conditions Assists patients in navigating the healthcare system and serves as a liaison to external behavioral health care, including specialists and other health care services outside of Coastal Medical Requirements: Licensed Independent Social Worker (LICSW), Licensed Clinical Psychologist, or Licensed Mental Health Counselor (LMHC) licensed in the State of Rhode Island and in good standing Prior experience working as a social worker in a medical setting such as a primary care office or hospital-based setting Any combination of education and experience that is substantially equivalent is also acceptable Why You Should Apply: We operate in a collaborative environment and strongly encourage and promote employee growth and development.
