General Function: Under the guidance of the Executive Director of Healthy Living, the District Director of Healthy Living will be accountable for the implementation of the YMCA's Diabetes Prevention Program, Parkinson's Exercise, Diabetes Management, Blood Pressure Self-Monitoring, Tobacco Cessation, LIVESTRONG at the YMCA, Healthy Weight & Your Child, and other evidence-based health interventions as needed; including daily operations related to the programs, provide support and guidance to other lifestyle coaches/program instructors, enter program data in a timely manner, and ensure that the programs achieve quality performance outcomes. Evening and weekend hours may be necessary at different times throughout the programs. Requirements: A commitment to the YMCA's mission and more specifically, the vision and goals of the YMCA's focus on health seekers. Must have outstanding communication skills; exceptional organizational skills; a passion for data and details; flexibility; and the ability to build strong relationships, inspire confidence and work effectively with a wide variety of stakeholders (health seekers, lifestyle coaches/program instructors, and all branch staff). Public health degree or experience preferred. Responsibilities: Successfully complete and pass all certification trainings required to implement evidence-based health interventions. Serve as the point of contact in the community for all potential participant inquiries about the evidence-based health interventions. Develop relationships with branch staff & members to identify program needs and program opportunities. Monitor healthy living email inbox to ensure timely response for inquiries. Provide marketing materials for branches/sites within the district. Participate in branch & community events to promote healthy living programs. Schedule & facilitate programs within the district. Maintain participant rosters with accurate information. Implement evidence-based health interventions bearing in mind the principles of behavior change theory. Provide print materials for all classes. Contact program referrals/inquiries within 72 hours of receipt. Keep track of contact in a log provided by the Executive Director of Healthy Living. With the assistance of the Executive Director of Healthy Living, expand programs to additional branch or community locations, as deemed necessary. Build effective, authentic relationships with program participants; helping them connect to each other and the YMCA. Enroll qualified individuals into appropriate programs. Maintain records as required for each program (e.g., weight, food/PA tracker status, and PA minutes at each DPP session; attendance, goal tracking, weight, measurements, etc.) Ensure accurate data collection and prompt (within 72 hours) data entry. Extract data from electronic data management system and prepare data reports for district and/or locations. Determine projected class schedule for each calendar year, with assistance from the Executive Director of Healthy Living & other District Directors. Assist with identification of and application for grants and community funding to support evidence-based health interventions. Assist with identification of community need for additional evidence-based health interventions. All other duties as deemed necessary by the Executive Director of Healthy Living. Why Work At The Y?
