Status: Non-Exempt Position Location: Great Livin ' LLC Facility Reports to: Great Livin ' Program Manager and QIDP Supervises: N/A Position status: Full Time Transportation: Required Job Requirements Summary : The Case Worker is an integral part of delivering high quality care to the individuals we support. Skills and Qualifications: A bachelors degree in a human services field . T he Case Worker must be goal driven and a self- starter and team player. Applies critical thinking to identify solutions to problems Must have computer skills- Must know or gain knowledge of Therap , Micosoft 365 and other supporting software. Key Areas of Responsibility: KEY AREA OF RESPONSIBILITY 1 : ICF Case Worker KEY AREA OF RESPONSIBILITY 2 : Acquiring and Maintaining community services for all enrolled individuals. KEY AREA OF RESPONSIBILITY 3: Quality Improvement and CQL POM interviewer Responsibilities include: ICF Case Worker: Admissions Committee Chairperson: Enters all applicants into Therap Conducts interviews with applicants and their supports Requests documents to complete application if needed and ensures they are submitted before presenting to committee. Presents to committee and gives recommendations Sends communication to applicants after committee meeting. Ensures all documentation is completed prior to admissions. Ensures all client services are current and maintained Supports clients with acquiring services and supports as needed, including mental and physical health, employment, education, social, and financial. Safety Committee Member Human Rights Committee Member Participates in IDT meetings for clients Acquiring and Maintaining community services: Assistance with applying for and maintaining: Social Security, Medicaid, Medicare, Community Benefits i.e. housing, food stamps, transportation as needed . Quality: Participates in Quarterly BA , QA Meetings and provides data as requested. Agrees to become trained as a CQL POM interviewers, conducts all CQL interviews with all clients throughout the year to ensure best quality care. Other Responsibilities and Duties: Must follow dress code per handbook and dress in a business casual attire at all times . Maintains clinically appropriate boundaries with clients, other employees and community member/IDT members. Other duties as assigned.
