DUTIES AND RESPONSIBILITIES: Provides PRS and / or CRS services to patients under the direct supervision of an Occupational Therapist and/or Director. Assists in the evaluation process and in identifying a problem list, short-term goals, long term goals and treatment plan. Conducts both individual and group services as assigned by the supervisor and outlined in treatment plan. Participates in patient, family and staff education. Confers formally and informally with other team members in coordinating the total rehabilitation program of the patient. Maintains patient records that reflect patient treatment: (Evaluation, daily treatment record, physician update, patient care plans, discharge summaries, telephone contacts, billing). Clinical knowledge and skill required for evaluation and treatment of the behavioral health out-patient populations. Ability to travel between facilities or other treatment locations as necessary. Responsible for performing tasks that are within the scope of his/her educational preparation and knowledge. Assists with program services for a client caseload by participating in activities which may include, but are not limited to: Gathering sufficient information for each client to make an assessment necessary to determine goals for the treatment plan or referral to the treatment resources in the community; Regularly assesses with supervisor the progress of the client's treatment and is open to recommendations for improvement. Helps consumer develop or regain physical or mental functioning or adjust to disabilities by implementing programs involving prevocational, vocational, education, independent living skills, activities of daily living, educational, recreational, and social activities. Promotes maximum independence by selecting and constructing therapies according to individual's physical capacity, intelligence level, and interest. Prepares consumer for return to employment by consulting with employers; determining potential employee difficulties; retraining employees; helping employers understand necessary physical and job result accommodations. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies. Flexible hours required. Participates in all required internal and external meetings. Provide PRS and other skill-related support to clients in the Community Residence setting as needed. Performs other duties as assigned. Assist Administrative Assistant with CQI tasks related to the program, include but not limited to: Monitoring participant data to track progress over time Share data with the individuals to whom it applies to facilitate partnered decision- making and to keep clients informed of progress as needed Work with the entire CN Guidance team with continuous quality improvement approach to address systemic, organizational challenges Complete QI Training during onboarding and participate in QI refresher training as needed EDUCATION AND EXPERIENCE: Licensing: Current state licensure or license eligible in states where applicable. Training and experience: Unless otherwise indicated, one year of current experience Recent experience working with adults diagnosed with mental illness. Experience in a prevocational or psychosocial rehabilitation setting preferred.
